Below is a list of California counties. Each is a link to a list of
facilities in that county. Within each county, facilities are sorted first
by city, then by facility name. Lists are
also available in PDF format.
In order to properly interpret this information, you should first read
through and/or print out the Services Key
and the Violations Key.
Notes:
- In the case of counties with few or no skilled nursing
facilities, a county may be grouped with listings for other nearby counties.
- Note that Los Angeles (LA) county encompasses over
400 facilites; therefore its listings have been split into five sublists
according to Department of Health Services Licensing and Certification
district office delineations.
- Normally these lists are updated quarterly, however,
various factors may affect timely posting of the data. At any time, please
feel free to call the CANHR office to obtain the most recent data available.
- Although they contain no graphics, County Lists range
up to 400k in size. Please note that larger lists may take several minutes
to load.
Complaint and Deficiency Data in these lists were updated from DHS data as of 2/25/2008.
Citation information is current for citations received by CANHR from DHS L&C district offices by 1/23/2008.
