Most people don’t want to think about their own end-of-life decisions ahead of time, postponing important healthcare and financial planning until it’s too late. A little bit of preparation can go a long way to help plan for future financial and medical needs. Advance Health Care Directives (AHCD) are meant to give adults control over their health care if there ever comes a time when they can no longer make choices on their own.

CANHR recommends that anyone filling out an AHCD review the choices with their health care provider and an attorney before executing the form.

A Durable Power of Attorney (DPA) for Finances is a relatively easy way for a person to plan for the future management of their financial affairs. A DPA can be completed by an individual who still has capacity to make financial decisions, in order to give someone else the authority to manage financial affairs.
