On 3/2/09, a resident became severely ill and died of cardiac arrest after a nurse failed to call 911 because she incorrectly thought the resident had a do not resuscitate order. The resident had signed a form stating he wanted life-sustaining treatment in case of an emergency. Due to the mistake, the nurse gave comfort care but not CPR and did not inform the resident's physician of the grave change in his condition. Paramedics were not called until a family member insisted after being notified by a friend of the resident's dire condition. Paramedics gave CPR once they arrived but were not able to save the resident, who died within an hour. The facility's failure to provide life sustaining treatment led to the resident's death.
