On 11/22/07, a 94 year old resident wandered from the facility undetected and was struck and killed by a car in the middle of a busy four-lane highway. The resident, who suffered from dementia, wore a Wanderguard bracelet, which was supposed to trigger an alarm if she attempted to leave the facility. Due to agitation, the facility planned to check her safety every 15 minutes on the evening of her death, but apparently didn't do so. According to the police report, a facility nurse claimed the resident was last seen in bed at 12:45 am, despite the fact the resident was struck by the car 12 minutes earlier. The facility was cited because the lack of supervision led to the resident's death.
